Submit registration on line below (preferred) or print out and mail/fax the form to Audrey Zelanko below.  You may pay either by credit card or check/purchase order.  Once you submit your registration form on line, a payment form will direct you to pay by credit card on line or print out a form and mail with payment by check or money order.  Please note registration fees below.  All registration fees must be paid prior to the conference.  Remember that hotel registration is separate. 

Registration fees are as follows: early registration $160 (ends 5/20/09), late registration $200 (starting 5/21/09), and one day registration for Tuesday only $75.  If you are a speaker and only attending the day of your session, no payment is due.
